Built around handoff, not charting
The blocks follow the order report is actually given: who the patient is, what brought them in, what is wrong now, what we are doing about it, what is attached to them, and what changed on your shift. Nothing on the sheet asks for anything you would not already be told.
The skin block is deliberately large
Skin is where a missed detail turns into a pressure injury nobody can date. The sheet gives you a front-and-back body outline to mark, a row of stages from DTI to unstageable, and six numbered rows so the mark on the body has somewhere to be described.
One page, measured, not guessed
The layout is checked against a real Letter page: one sheet, ten millimetres of margin on every side, ruled lines drawn as borders so they print even when your browser has background graphics switched off. The builder shows you how many millimetres are left before it spills over.
Nothing on it identifies this site
The printed sheet carries no logo, no address and no branding, so it is safe to carry on the floor and to hand to a coworker. The only line on it that is not yours is the reminder that it is not a medical record and should be shredded at the end of the shift.
It never leaves your device
There is no server behind the builder. Your layout lives in your own browser and in the file or link you choose to share. Your unit logo is read locally and never uploaded. Nothing you type is sent anywhere — which also means you should never type a patient's details into it.
Free, and staying free
Print as many as you want, for yourself or the whole unit. No sign-up, no watermark, no trial. It exists because the show is about this job, and a good brain sheet is more useful to a nurse than another poster.
Questions
What is a brain sheet?
The single page a nurse carries all shift with everything about their patients on it — report, times, lines, what is still pending. Some units call it a brain, some a report sheet, some just “the paper”. It is not a legal record and it does not go in the chart.
Will it print on one page?
Yes, on US Letter at 100% scale. Do not let the print dialog shrink it to fit. If you build your own, the counter at the top tells you before you print whether it still fits.
Can I change it for my unit?
That is what the builder is for. Move blocks, resize them, rename any heading, add your own checkbox rows and tables, and put your unit name or logo at the top.
Can I share the one I made?
Press Copy link in the builder. The whole layout is packed into the web address itself, so whoever opens it gets your exact sheet. No upload, no account, nothing stored on our side.
