Who this sheet is for

Two patients, half a page each. It is for a lighter assignment, or for anyone who finds four-patient sheets too cramped to write on.

Use one half per patient and keep the same order on both. The top lines are for report: room, allergies, diagnosis, diet, activity, lines and drains, and the code and safety boxes. The middle row is where the shift happens: vitals up to four times with glucose, meds by the time they are due, and labs or tests you are waiting on.

Tick the to-do boxes as you go. Before you hand off, fill in intake and output and what changed, then use the notes lines for what the next nurse needs to hear first. The sheet folds in half, one patient per side.
